The Central Records Office is proud to
serve the residents and visitors of the Town of Falmouth. We hope to
assist you in making your experience with us as comfortable as possible. We
process approximately 25,000 calls for service a year, therefore, due to the
large number of requests, we ask for your cooperation. We will do our best to
serve you in a timely manner.

Requests for incident or MV accident reports may be made
in person, writing or by phone. Criminal offender information must be
picked up in person by the offender with proper picture identification.
Reports may be requested by mail by filling out a Report
Request Form and mailing same to the Central Records Office with a self
addressed stamped envelope. You will need Adobe Acrobat Reader to
view/print this form. If

Applications for FID cards and Licenses to
Carry firearms are processed by the Central Records Office.
Application/information packets may
downloaded or be picked up in person at the police station. In order for an
application to be processed, the applicant should bring the completed
application and a check/money order for $100.00 made payable to the Town of Falmouth to the
Central Records office between 8:30am and 3:30pm. The
Central Records office does NOT accept cash. [Top

The Commonwealth of Massachusetts requires any operator of a
motor vehicle involved in a crash to file a
Motor Vehicle Crash Operator's Report within 5 days
whenever someone is injured, killed or there is damage to vehicles or other
property in excess of $1,000. This report should be made out in
triplicate, with the original sent to the RMV in Boston (address is on form),
one copy forwarded to the police department in the city of town where the
accident occurred and the operator should keep one copy for his/her insurance
company. If

Disabled Placards or License Plates are only available from
the Massachusetts Registry of Motor Vehicles. The application can be picked up
in person in the Central Records Department, or can be downloaded here in Adobe
Acrobat format. If

returned to the Medical Affairs Bureau of the Registry of Motor Vehicles.

Town of Falmouth by-laws require that all residential and commercial burglar
alarm systems be registered with the police department. Registration is
free and must be updated annually. The
alarm registration form may be
picked up at the police station or downloaded. Please mail completed forms to the Central Records
Office, 750 Main St., Falmouth, MA 02540 or fax completed forms to

Taxi Permits
Individuals requesting a Town of Falmouth Taxi Operators permit must apply at
the Central Records office. Applicants will need to have a letter of
employment, two (2) 1"x1" color photos and a check or money order made
out to the Town of Falmouth in the amount of $25.00. [Top

Hawker/Peddler Permit
Individuals requesting a Town of Falmouth Hawker/Peddler permit can pick up
an application for the Commonwealth of Massachusetts Hawker/Peddler permit at
the Central Records office. Once the state permit is issued, the applicant
will need to return to the Falmouth Police Department and register to conduct
business in Falmouth. Local registration requires that the applicant have
two (2) 1"x1" color photos and a check or money order made out to the
Town of Falmouth in the amount of $10.00. [Top

Parking permits for the village of Woods Hole are available for Falmouth
residents and property owners from the Central Records office. A current
vehicle registration (with Falmouth address), property tax bill, or other
suitable identification will be required in order to obtain a permit. Cost
for the parking permits is $10 per vehicle. The permits are issued
annually, expiring June 1 of each year. [Top
